The Arizona Wellness Center for Women is located at 3811 East Bell Road Suite 212
- Phoenix, Arizona 85032

- We are near the corner of Bell Road and 40th Street next to Paradise
Valley Hospital. We are providers for Mercy Care, APIPA, Phoenix Health Plan and Health
Choice. We deliver our patients at Paradise Valley Hospital and Scottsdale Healthcare
Shea.
Baby Arizona Mission
To increase the percent of healthy babies born to Arizona women and to improve the health
of expectant mothers through a public/private partnership that focuses on outreach to
low-income pregnant women and reduces barriers top early prenatal care.
Requirements
Only women who are U.S. citizens may participate in the Baby AZ application process".
We are not able to process applications on someone who is a legal refugee, asylee, or
lawful permanent resident.
